Care Coordination
Intensive care coordination is offered for adults and children with a severe mental illness that substantially interferes with the person’s ability to carry out daily tasks. Case managers provide linkage to appropriate services that help individuals function optimally within the community. Resources include assistance with housing, counseling, basic life-skills training, physical health, education, employment and social/recreational opportunities. Services are conducted in the office, in the clients’ homes or in their communities.
Outreach/PATH (Project for Assistance in Transition for the Homeless)
Outreach, referrals and supportive services are available to individuals in Orleans Parish with serious mental illness, including those with co-occurring substance abuse and mental health disorders, who are experiencing homelessness or at risk of becoming homeless. PATH services include community-based outreach, referrals to resources for mental health and substance abuse treatment, service coordination and limited housing services.
Psychosocial Rehabilitation Services for Children and Adults
Behavioral health counseling, consultation, and referral services are available for children and adults who may be facing particularly difficult and challenging life situations. Intensive case management and therapeutic in home, in school and community-based interventions can be provided to successfully address mental health, relational, co-occurring and/or behavioral health disorders.
